An exciting opportunity has arisen for an experienced and motivated Registered Nurse to join our supportive and dynamic Care Home Project Team. The team is committed to providing high level facilitation to care homes in North Brent area. You will alongside the Team Leader, GP’s, and care home staff.
You will empower care home staff to proactively identify residents in care homes entering the last year of life, assess and plan for their identified needs and respond to the residents changing condition.
You will support care home staff to identify training needs in relation to End of Life Care and support them to access appropriate programmes of training. You will be required to deliver programmes of education in care homes and work collaboratively with the Education Team at St Luke’s Hospice
You should be an experienced Registered Nurse ideally with community nursing or palliative and end of life care experience. You will possess excellent clinical assessment skills and be able to work autonomously in order to deliver a high quality service to end of life care patients in care homes. As a key member of a small team you will be a good communicator who can work flexibly in a team to meet the needs of the service.
The service operates between 08.30 – 4.30 pm, Monday – Friday. Car driver and owner essential. We offer a frequent car user and competitive mileage allowance.
