Referral Criteria and Process

Due to limited resources we are only able to provide services to those adults in Harrow and Northern Brent with:
Advanced progressive illnesses which are no longer responsive to cure and either have such complex needs as to be best met by a specialist palliative care service or have an expected prognosis of less than one year.

We would hope to influence the care of patients who we are unable to help directly, by our education and training services.

Any healthcare or social work professional may refer a patient to the hospice. Informal enquiries from a patient, carer or a member of the public are welcomed and appropriate advice will be given.

Decisions on suitability and prioritisation of service provision, will be made on clinical need.
