Daycare services aim to support patients with advanced or progressive illness who remain in the community, liaising with their primary care practitioners and nurses to provide a holistic and person centred care package.
Day Care is able to offer: -
- Daytime Respite opportunities for carers, relieving some of the stress associated with this role
- Symptom management for patients with advanced, progressive disease
- Symptom management for patients with complex needs
- Opportunities for patients to receive blood transfusions or other infusions without the need for admission
- Person-centred care using an interdisciplinary model (to include the patient) to establish need and formulate appropriate care plans to meet the identified needs of the patients
- Social stimulation
- Family support
- Complementary therapies
Like the inpatient unit, patients have ready access to nursing staff with further access to doctors, physiotherapy, speech and language therapy, social work, a spiritual care coordinator and complementary therapies as appropriate and subject to availability.
