Care at Home

St Luke's Hospice offers many of its services to patients in their own homes, enabling them to remain in familiar surroundings rather than being admitted to hospital or to St Luke's inpatient department.

Our services come in various forms:

The Hospice at Home team covers North and South Brent and Harrow and provides personal care packages (day and night) which may include lifting, washing, transferring, toiletting, as well as much support to carers. The rapid response enables patients to remain at home or be discharged rapidly from hospital or hospice to spend as much time as possible at home and to die there if that is their wish. Read more

The Specialist Community Palliative Care team for the North Brent area provides support and advice to patients, and also to theGPs and district nurses who have overall responsibility for managing patients in their own homes or in care homes. In addition the team provides skilled care to patients who wish to remain in their own homes, but who require a greater level of care than is available from district nurses or their formal or informal carers.
