Community Specialist Palliative Care Team Manager    

St Luke’s Hospice strives to provide the highest quality palliative care to the community it serves. The Specialist Palliative Care Team provides advice and support to patients living in the borough of North Brent, including Kingsbury, Wembley and Kenton.

The team provides a responsive service, offering specialist palliative care advice including the ability to deliver safe evidenced based symptom management, alongside the provision of social, emotional and spiritual care for the patient and family. You will work closely with, and act as, a resource to primary care teams and other care providers with a common aim of achieving the best quality of life possible for our patients.

We are looking for an enthusiastic and highly motivated nurse, to assist in the continued development of quality and achievement our strategic aims. For example; as the service lead you will be key enabler, to support the development of a 7-day provision.

Post registration experience/qualifications in palliative care or related specialty, highly developed communication skills, the ability to practice autonomously and experience of managing Clinical Nurse Specialists (or similar) are essential.
