Job summary
The Home Care Sister will work with and assist the Home Care Manager in providing Community Services commissioned by St Luke’s Hospice ensuring a safe and responsive service. The team works together to prevent unnecessary admission of patients to acute or residential settings, when their preferred wish is to receive care and support in their own home.
The Home Care Sister will provide leadership, management and support all staff within the Home Care Team at the Hospice and participate in the planning of further development of the service.
