Advance Care Planning Volunteer

Accountable to: Director of Patient Services

An exciting opportunity has arisen here at St Luke’s Hospice to join our Wellbeing Team as an Advance Care Planning Volunteer. You will be working within the local community helping those who may be frail, elderly or have a life limiting condition to access the ACP service and encourage them to document their wishes and decisions with regards to their care and support needs.
If you are well organised and flexible with a compassionate and friendly nature, the role of Advance Care Planning Volunteer could be for you.
You will receive the appropriate training and as you may be required to travel for this role to locations such as care homes and GP surgeries, expenses will be paid.
Please note this is an unpaid volunteer role.
Support & Personal Development:-
- You will receive training for this role from St Luke’s Hospice and/or one of its partners, which will furnish you to carry out the role and may also provide personal benefit.
- With the above in mind, we ask that you make a commitment to carry out the role for at least 6 months after training. A minimum of one day per week equivalent would also be preferred.
- We require that you attend regular support and supervision sessions including reflective practice.
- We also require that you contribute to the development of curiosity and inquiry which seeks to maximise the opportunity for continuous learning within the service.
As a St Luke’s Hospice volunteer it is your duty to abide by any relevant code of conduct and / or practice applicable to you.
