We help people in Brent and Harrow to have the best possible quality of life despite living with serious illnesses, which are no longer curable.
You may have an advanced cancer, or a long-term condition like heart or lung failure, a progressive neurological disorder, or another end-stage illness. Whatever your illness, our team aim to make life better for you and your family.
We have a range of specialist health and social care professionals who will work with you and your family to find out how you would like to be cared for. They will give you information and will discuss your preferences and plans, ensuring you maximise the quality of the last phase of your life.
Our range of services are provided 365 days per year and can be provided in your own home, as an outpatient or within our 12-bedded inpatient unit. Our services are free at the point of contact.
Referral Criteria
To access our services you will need to meet the following criteria:
- Be living with, or affected by advanced disease
- Require specialist help to address complex symptoms, psychological/emotional support or end of life care.
- Give your consent to use services and share information with other professionals
- Be 18 years or older
- Living in Harrow or North Brent
