We understand it is difficult when you lose a loved one. You may find comfort in creating a personal Forever In Our Hearts Tribute Page in honour of their life. Your online page will give you a positive and lasting space to add messages, photos, music and videos to celebrate their life.
You can choose who you would like to share your tribute page with, and invite friends and family to leave personal messages in your loved one’s memory. People are also able to make a donation to St Luke’s if they wish. St Luke’s Forever In Our Hearts tribute pages are run in partnership with, MuchLoved to provide our supporters a free service to remember someone special and fundraise in their memory.

How to set-up a tribute page
To create your tribute, please fill in a few boxes below. You will then receive an email to activate your page and then upload a photo (optional), a message and your personal memories. You can then share your unique tribute link with family and friends.

Does my tribute page have to be public?
Your page can be kept private with special access to selected family and friends; or you can set it to be public to allow anyone to pay their respects. The privacy of your tribute page is in your control and you can amend it at any time.
How long does my tribute page remain open?
Your tribute page can remain open for as long as you wish. You can choose how often you want to contribute to the page, use the features to remember your loved one and share it with others.
How are donations collected?
In a similar way to sites like JustGiving, donations raised through your page are collected by Muchloved.com. Donations are automatically transferred to St Luke’s bank account. You can use your tribute page to let people know about fundraising events that you may be taking part in. We can also add offline donations and event sponsorship to your tribute page.

Would you like grief support?
St Luke’s bereavement support team offers confidential, individual support to help cope with the loss of family or friends. The support is available to anyone who has experienced the death of a loved one who was cared for by the Hospice.
