Did you know that in 2024/25, St Luke’s Brent Community Specialist Palliative Care team completed 2,072 visits and 5,826 calls to support 610 patients, enabling 74% of these patients to die in the place of their choosing?
Care in the community is what most of our patients and families want.
St Luke’s Hospice and you make this possible thanks to your donations and the expert support of our highly trained palliative care nurses who work every day from 8 am until 8 pm to ensure their patients and families living with life-limiting illnesses have access to individualised support that makes their lives a little easier.
Charlotte, one of our Community Clinical Nurse Specialist in Palliative Care, says,
I enable and support their wishes through our community palliative care services, organising specialist equipment, managing symptoms with medication and holistic care to help them cope with the challenges of their illness.

Our care extends beyond our Hospice walls
Many more of our patients living with life-limiting illnesses wish to remain and then die in their own home, surrounded by family and friends and avoiding unnecessary hospital admissions. They want to keep their close-knit family time and sleep in their own bed!
Where we can, we enable and support their wishes through our community palliative care services, organising specialist equipment, managing symptoms with medication and holistic care to help them cope with the challenges of their illness.
Your donation will make a huge difference
Our community nurses provide personalised care that goes beyond medical intervention. They listen to their patients and families, thoughtfully assess the situation, and offer options that help reduce anxiety and resolve challenges.
Our feedback from family members shows the difference their special care makes.
“Thank you, St Luke’s, for keeping my mum comfortable at home… Your team were proactive and on hand whenever needed… Thank you for taking care of my mum’s needs and our emotions at the time.“
“The nurse’s careful assessment allowed my husband to have more independence and all the pain relief he needed.“
“The nurse has been the beacon of light… enabling us to make informed decisions.”
“I couldn’t have carried on without the carer support four times a day and the 24-hour helpline St Luke’s community care team provided.”
Please donate today
We rely heavily on your donations to provide our community care, as it is only partly funded by the NHS. Your support through donations will help ensure Charlotte and the Brent Community team can deliver the right care in the right place.
Every donation will help towards funding our personalised nurse-led palliative care in the community. Every donation will make a difference.
