Community Palliative Care Nurse / Community Palliative Care Paramedic (Development Opportunity)
Contract: Permanent / 37.5 hours per week
Hours of work: The post holder will be expected to work as part of a provision of a 7-day service and may be expected to work as part of an 8am-8pm rota as part of service development in line with the NWL ICB New Model of Care.
Salary: £36,453.76 – £43,896.54 plus London allowance depending on experience
Location: HA3 0YG

Job summary:
This is a unique development opportunity for a registered nurse or paramedic to grow into a Clinical Nurse Specialist (CNS) or Community Paramedic Specialist role. Working within our Community Specialist Palliative Care Team, you will play a pivotal role in delivering holistic, patient-centred care to individuals living with life-limiting conditions. You will work closely with our team, supporting patients, their families, and carers with clinical and emotional support.
Key Responsibilities:
- Support community healthcare providers with complex palliative care management.
- Act as a keyworker for patients, conducting holistic needs assessments and developing tailored care plans.
- Provide specialist palliative care, delivering high standards of physical and emotional support.
- Manage a caseload efficiently, liaising with healthcare professionals and community teams.
- Engage in multidisciplinary meetings, service development, and training.
- Maintain accurate patient records in line with Hospice policy.
